PrimaryPlus knows and understands that the word DIABETES can be scary and very unsettling. No matter how many years you may have had the disease—those thoughts and concerns never really go away. Through our WeCare® program, we can help ease your fears of diabetes by helping you to better understand the disease and give you tools to manage it. Our program is centered around helping you control your diabetes. WeCare® is composed of an amazing group of cheerleaders and coaches (aka Nurses, Medical Providers, Pharmacists, Dietitians, Certified Diabetes Educators and more) to help guide you through this journey to a HEALTHIER YOU!

Being Part Of The WeCare® TEAM Means…
  • Having your very own Case Manager that can help coordinate diabetes services, be there to offer support and answer questions whenever they may arise.
  • Being provided with a Care Tool Guide that helps keep you on track with your care such as referrals, testing, immunizations, etc.
  • Having access to a knowledgeable, well-trained team of diabetes experts including Medical Providers, Pharmacists and Registered Dietitians to empower you.
  • Many diabetes screening opportunities available on-site (such as Diabetic Retinopathy Exams) at PrimaryPlus to keep your diabetes care close to home and as convenient as possible for you and your family.

WeCare® Case Management

The Case Managers for PrimaryPlus’ WeCare® program serve as the tour guide on the diabetes journey helping patients understand their health, the opportunities, the resources and the benefits. Their nursing background provides them with all the right tools to help you achieve optimal health. It is our goal to work with the patient on many levels to close care gaps and be a direct line of communication.

Medical Nutrition Therapy

PrimaryPlus’ Registered Dietitians are excited to help educate patients on ways to have a healthy relationship with food.  They provide Medical Nutrition Therapy (MNT) at most PrimaryPlus locations. They believe in helping you assess your diet–teaching you to make REALISTIC changes.  These recommendations will be tailored just for you based on a variety of factors while also taking into account your current eating habits.

Medication Therapy Management.

PrimaryPlus wants to be a partner in your health, connecting the pieces of your visits from start to finish. Through the Medication Therapy Management program, PrimaryPlus offers one on one appointments with our skilled pharmacist, Danielle Eisenhauer, PharmD, CDE, MLDE. Danielle provides assistance in best understanding your medicines that are prescribed by your doctor. The visit consists of…

  • Learning how the medication works.
  • Discussing how it is taken best for effectiveness.
  • Reviewing medication for appropriateness.
  • Assisting with any updates to medication profiles.
  • Working with the medical provider if any issues.

Patients will receive a pocket medication list, as well as, the opportunity to be given any immunizations that may need to be updated, PLUS any additional education materials that help to best understand their medications
