Ashland

The PrimaryPlus-Ashland office opened its doors on September 22, 2014. This office originated as a new, exciting adventure and collaboration—one of the first of its kind in the state of Kentucky–where PrimaryPlus, a Federally Qualified Heatlh Center, was located inside the local health department.  While this collaboration was successful, it also left little room for growth.  In April of 2019, PrimaryPlus-Ashland made the move to 432 16th St. located in the heart of downtown Ashland.  The move brought about a large expansion project for PrimaryPlus-Ashland that included the addition of x-ray and ultrasound services, added medical providers, an on-site pharmacy, and additional exam rooms.  In the summer of 2020, PrimaryPlus grew its pediatric team and even created a separate pediatric entrance on 16th St. (with the main entrance being located on 17th St.).  As part of PrimaryPlus mission, we want to make healthcare accessible and affordable; therefore, we continue to grow and add new services to best serve the region.  

PrimaryPlus offers a complete range of services for the entire family including family health, pediatrics, women’s health, counseling and an on-site pharmacy that is OPEN TO EVERYONE.   Additional services include Osteopathic Manipulation Services by some of our family health physicians.
