We work to provide as many opportunities to keep your medical expenses to a minimum whether it be bringing specialty services close to home through healthcare collaborations to minimize travel expenses or making our pharmacy savings readily available or extending our Sliding Fee Scale to those that have an inability to pay. PrimaryPlus accepts most commercial insurance, Medicare, all KY Medicaid plans, OH Medicaid (CareSource & Molina) and offers a Sliding Fee Program that can provide up to a 75% discount on care for most services.
As a Federally Qualified Health Center, PrimaryPlus is proud to offer a Sliding Fee program for those that are uninsured or underinsured (examples include high deductible plans, Medicare gaps, etc). The application process is easy and once processed; households can qualify for up to a 75% discount on care (maximum slide percentage varies by service type). Sliding Fee Applications can be picked up at any PrimaryPlus location or printed below.
Note: Applications need to be filled out, listing all individuals living in your household (discount will apply to all living within the home). Proof of income will also need to be provided with the application. Please include proof for each individual earning income in the household.

PrimaryPlus is a proud participant of the Kentucky Prescription Assistance Program (KPAP). The basic concept of the program is to provide patients who have NO prescription insurance coverage the opportunity to receive their medications for free or at a reduced cost (this initiative utilizes existing assistance programs offered by pharmaceutical companies).
