PrimaryPlus-Bracken: Community Connection
In June of 2009 the Bracken County community welcomed the opening of our PrimaryPlus-Bracken office with open arms. That “hometown” touch is very present in the Bracken office. Considering that much of the staff grew up within the community, there is a great sense of comfort and familiarity. Services within this office include family health, pediatrics, x-ray/ultrasound, sports medicine, minor procedures and an on-site pharmacy. We are also humbled to offer our trademarked WeCare® program at this facility utilizing a team of Medical Providers, Case Managers, Pharmacists, and more to give a TEAM approach to chronic disease management.
In early 2023, PrimaryPlus, the Bracken County Health Department, the Bracken County Board of Education, the University of Kentucky College of Dentistry and Interact for Health partnered on a grant project to build a school-based health center in the Brooksville community which is just about 10 minutes from the PrimaryPlus-Bracken County campus located in Augusta. This new location houses dental services, as well as, family health. While the building may be small in size, it has made a BIG impact on the health of the region. The dental center offers state of the art equipment with a medical and dental team that BELIEVE in this community. This project is a perfect example of the POWER OF COMMUNITY & CONNECTION. Although the name says “school-based” health center, this location is actually open for the entire community and all ages to utilize. The dental team visits both schools within the county on a regular basis for routine check-ups and the primary care services are available to both schools via telemedicine visit and in-person care.
Collaborations:
PrimaryPlus-Bracken County welcomes the St. Elizabeth Mobile Mammography unit on a regular schedule to help with access to mammogram services.
Affordability
PrimaryPlus accepts most insurance, Medicare, all Kentucky Medicaid plans, OH Medicaid (CareSource & Molina) and offers a sliding fee scale for those that have NO insurance and/or are underinsured. We are excited to pilot our NEW Online Scheduling program. This is available for most of our primary care providers at this location.
