Preparing for your visit

We are always working hard to improve and simplify the registration process for our patients. We offer a digital check in process; you no longer need to fill out endless pages of paperwork. Using your phone, tablet, or computer, you can sign documents, fill out forms, pay your co-pay, and much more. You will receive an email and text message before your appointment simply click on one of the links and you will be able to complete the registration process at your convenience, including your medical histories. This helps save time for you and our medical staff, ensuring you get the most from your visit. Once you arrive at the office you will only need to alert the front desk staff that you have arrived, they will then make sure everything is in place, (some time sensitive screenings may need to be completed upon arrival) and you will be ready to begin your visit with your provider.

Items you will need at your visit:

Need: ID & Co-pay
You will need to bring the following on the day of your appointment: driver’s license/photo ID, insurance card, medications, and co-pay. If you are uninsured or are underinsured and would like to apply for the sliding fee/discount program, please bring acceptable proof of income.
Consent for Minors
If you are a minor or under guardianship, your parent(s)/legal guardian(s) must sign the registration paperwork. At the first visit, parent(s)/legal guardian(s) can sign a form listing others who may bring the patient to their appointments.
Guardians: Bring Paperwork
If you are a guardian, have custody or power of attorney of said patient, you must bring your paperwork to the appointment with you. The patient cannot be seen without this paperwork.
Have a DNR? Bring It
If you have an Advance Directive/DNR or living will, you will need to bring this paperwork with you as well. It is your responsibility to provide this information to us.
List of Medications
Bring a list of all your medications with you to every appointment.
